Multistep Form using PHP, AJAX & MYSQL
Budget: $250 – $750 USD
The multistep form will be used to provide an automated home valuation. Code must be clean and commented, it is a simple project, do not use frameworks. Website must be responsive.

Please provide 4 different home page designs. The multistep page could be the same.
I will pass a variable via URL like ?a_id=15 that will be used in the last step.
Job description:
Step 1:
The "Enter your address" input field must integrate with Google Places API to display street addresses in the US. On Address selection, move to next step, it must be a NEW url, exactly as in the referenced website.
Step 2:
Use Google APIs to display map of the user's address (which will be received via URL), input field to enter Unit # is optional and if filled must be recorded to MYSQL. Also load variable a_id to be used in last step.
Steps 3-4-5:
Use same questions and answers, same button layout. On click, record input to MYSQL and display next step.
Contact Step: Use same contact questions, same layout. Validate email and phone. On click, record input to MYSQL.
Final Step:
After the form is submitted, redirect to a custom url + ?a_id=15 which will load an agent profile from MYSQL table (please come up with some design that looks great and is also responsive, I dont have any reference)
The Agent Profile page must contain.
1. An avatar photo
2. A cover
3. Name
4. Position
5. Company
6. A call me now button
7. Social Buttons
8. An about me text.
9. Custom Links (up to 10)
