Multi-page form to be updated to modern, single-page, responsive form

We have a multi-page form implemented in PHP that needs to be modernized. The end-product will be a single-page form that is secure and responsive. Project specifications are outlined in the project doc: https://bit.ly/php-freelancer-modernize-form

Budget: We are focusing on proposals in the $350 - $400 range. Proposals above $500 are currently not within budget and will not be viewed.
