Modernize current PHP data entry form with new framework

We have an existing PHP form that accepts data input and inserts it into two tables. It also displays any data previously entered to allow for modification*.
-- “Orders” table receives all data except what is entered in the “Notes” field (see below for field description)
-- “Notes” table receives anything entered into the Notes field on the form. Insert includes:
o Entered “Note” text
o User name of the person entering the data (derived from GET parameter in URL)
We wish to modernize the current data entry screen with a framework that uses Bootstrap 5. The current data entry screen functionality should remain the same with some changes that will be available using the new framework. Here are the changes that will be implemented using the new framework:
1. Look and feel will be modernized using the available styling available in the framework. The new layout should be as close to the original as possible with 2 columns
a. column1 = date entry fields
b. column2 = previously entered note history
2. Validation rules the framework allows, will be implemented (i.e. required fields, etc).
3. Conditional logic the framework allows, will be implemented (i.e. if “Has Contract” = Yes then show/unhide “Contract Number” field)
