Medical Alert Information ID addition to existing evaluation/tracking system.

We need to create a medical alert information ID badge for the consumers to wear when out in public in case of any emergencies, so first responders could know critical information... but they don't want the consumers medical information easily seen by just anyone.
We need to create a spot with the consumers in the program you already built, to add the consumer's picture, name, date of birth, address, any medical alert information needed, allergies, verbal/non-verbal, emergency contact information, alternate emergency contact information, guardian's contact information, Medicaid number.
That would create a 2.125 inch x 3.375 inch image with the consumer's photo, name, and a QR code that could be downloaded as an image for the consumer's ID.
The QR code could be scanned by first responders, which would take them to that consumer's critical medical information page, which is produced by the information entered.
As discussed:
1. The ones who could see this information would be anyone who would scan the badge, such as ambulance/emergency personnel. The medical information is not necessarily private. It would be things that would be on a medical alert bracelet or pendent that emergency personnel would need to know in the event of an accident, such as allergies or if the consumer has seizures.
2. Each consumer's information will be different, so the fields needed would be input fields so the information would be inputed manually. The date of birth could be either a simple date selection field, or manual input field. The Verbal/Non Verbal could either be a radio button selection, or a manual input field.
3. The current system has a list of consumers where currently the editable fields are the consumer's first and last name. Yes, if possible, we would to add the mentioned medical information here on the edit page, with a link that would allow the admin to download and print the compiled card in PDF format. Yes the card would need to look like the one I sent.
When the QR code is scanned, it doesn't need to take them to this page of editable information. This is in a password protected area of the program. Only staff who have access to the system would need to be able to access this page.
The scanned QR code would need to go to a separate page outside of the password protected area for that consumer, generated by the information put in on this editable page, without affecting the functionality of the current system. I am attaching some samples of what the current consumer pages looks like and a sample of what the generated medical alert information page might look like.
