Make My Website Form Working

We have a website and we want to hire someone who can help me to make its form working
Our order form has 12 fields and users can fill it in 2 steps
In Step 1, 4 fields are visible to the user, i.e, Name, Email, phone no., Date
and, in Step 2, All the 12 fields are visible.
So, once the user fill and submit step 1, s/he got redirected to step 2 and all the previously filled information gets copied in step 2. and he just have to fill the rest of the information.
After the user fill and submit Step 2, a unique order id is generated and sent to the User as well as to me.
You also have to add a Google captcha in Step 2.
Users should have the privilege to upload multiple files (Doc, image, pdf etc) along with the order form.
