Make A Google Maps Locations Page (PHP + MySQL)

Hi. I want a basic core php script made on white html page to list some locations from a MySQL database (I'll give you a copy of the table).
1. Show all the locations as markers
2. Detect the user's location with the allow pop up and show the user marker too
3. List the locations on the left by nearest (so show the distance in miles from the user) with the Address (from the db/table under the Location Name). I do have an existing feature that does this, so if you want to see the code or reference it that's fine.
4. Add a search field for typing in an address to sort the locations by nearest from the typed address.
Similar to this (but even simpler):
