Laravel form - Field changes

I am looking for a Laravel developer to make changes to my form's fields. The current functionality of the form is that it submits data to a database. I have a specific list of fields that need to be added or removed. The fields that require changes are input fields, dropdown fields, and checkbox fields. Ideal skills for this project include experience with Laravel and proficiency in PHP and HTML.
