I am seeking someone to create a form and build a form-builder in Laravel. This module will be added to the current/existing custom-built CRM which is in Laravel Framework.
1. You need to create/duplicate the form of the following in Laravel.

2. The form-builder must have functionality such as a conditional question. You can refer to the above link and select different visa types.
3. Any data collected from the user should be able to save in the current CRM.
4. You need to show it on your computer first and then we might give you GitHub access to commit the code once finalised.
5. The design of the form/page should have attractive design.
