Laravel CSV file upload in database using program

I am looking to create a program to upload a CSV file into a database using Laravel. This project is for my own personal use. I have previous knowledge of the Laravel framework, however the project must be based on the criteria provided in the attachments and must be completed within 4-6 hours!
