Interactive HTML (PHP) map based on Shapefile (.shp)

The map of Poland divided into 314 districts is in the .shp file. Each district is identified by unique identifier (TERYT). In the MySQL database there is a table where for every TERYT identifier there is an integer number and the URL assigned.
The PHP script has to draw the map where every district marked with a different saturation, depending on the integer from the database. The map need to be on the layer over the Openstreet or Googlemap (like on the page https://prokulski.science/blog_iframes/python_mapy/bezrobocie_gminy.html). After cliking on the district, there should be a small text information (the integer number and the link to click). That's all.
