Stripe collects:
Name
Email
Phone
Billing Address
Bank ACH Debit
Note that Stripe ONLY collects bank ACH information, no credit cards, so I need this form custom made to collect this information through Stripe.
After checkout is complete, it should redirect the user to http://1cop.com/next-step
