Initial first screen , of a potentially larger project

All code needs to be clearly documented and delivered for review prior to payment.

System will be developed using : Windows 10, Sql Server Express ,PHP,APACHE SERVER
1.- Log in and create new user screens:
using a Https secure connection;
Required fields:
User (email)/password (minimum requirements) 8 chars (minimum), 1 cap, 1 lower case, 1 numeric,1 sign;
Country,
Full name,
City, State (dropdown list) and Zip;
Mobile phone #;
Optional fields in the create user screen:
Full address
Social security number; (Tax Identification)
Functionality required:
Confirm email and phone number, sending an email, and sending a text to be confirmed within a period of time;
recuperate password function; (require text via cell phone recuperation)
remove user (per user request)
Terms acceptance check box and link to text;
Store all the data in a SQL database, using SQL stored procedures to perform the data store and/or modification;
Log in/Log off screen based on above;
Inactive time on screen automatic Log off;
