Hotel web check in form

I need a secure php-MySQL form that we can send to our guests a couple of days before they arrive to our hotel so that they can check in before they arrive.
The form will also collect passport number and date of birth so the DB and the submission need to be bullet proof.
Also I need an easy way to access the data, other than login in to PHPmyAdmin, preferably a page where I can login and download an Excel file.
