Help Importing an XML file to a database using MyPhpAdmin

Attaches are 2 files of the same document. One is in a PDF format, the second is in an XML file. In this project I am wanting to import this XML file to a table in my database
Looking at the code and format I am unsure what fields need to be added to the table.
I am creating a MYSql database using MyPhpAdmin
Requirment #1
Provide me an SQL code that will create a table with the needed fields. (SEE ATTACHED IMAGE)

Provide me instructions for successfully uploading the the XML file to the database

Team View into my pc and offer support if I need assistance during installation

If support during installation takes longer then 2 Hours, then I will pay your hourly rate for continued support until we get successful import
