Health Calculator using Code Ignitor
Budget: ₹1,500 – ₹12,500 INR
Patient will be presented multi choice question with multiple Slides based on option selected next question will vary.
Question answer format will be multiple selected with check boxes , single selective .
Each question will have individual score but the nature of the answer will be (+) and (-) Both.
Suppose : If you smoke, score will be (-10)
Based on customer option selected sum will be generated
Report with graph and Percentage based on score of final result . This final report page will contain- advice , suggestions, graphs, % based on options selected. The advice and suggestion formula will be given to you.
REQUIREMENT-
1. QUESTION MANAGEMENT ( ADD/EDIT/DELETE)
Developer Suggestions-
While creating questions and their options, each answer will have options to bind multiple suggestion and score whether its + or -ve .
Eg- Add question, Add option, Add option type drop down, For multiple selection check box, For single selection radio button.
Add Unlimited Option with + button. Option field will be generated
Answer Suggestions.
If user selects these options , following suggestions will be shown.
Every answer will have option to add suggestion.
FINAL Score Nature ( + or -) , Then score values .
2. SLIDE MANAGEMENT
Journey of user calculator will start from slide one.
Each slide will contain 1 question.
4 options will come. If 1 question answer is x then second question what will come this will the formula . Based on answer selected on previous slide, next question will come.
Multiple Slides , Then Final Slide closure.
Frame work- Code Ignitor 3
Note- Front End Calculator will be presentable on multiple slides and should be interactive. Use java script or jquery .
