HTML Coding for Dynamic PDF output - REPOSTING

I need single page HTML which should be compatible mpdf (https://mpdf.github.io/) or dompdf (https://github.com/dompdf/dompdf) conversion. Based on birth date & fullname, we will send PDF files to user, so PDF should have dynamic content (we will use libraries above to convert html). Therefore I need you to create single HTML page.
