Google Form Integration to Google Contacts

Simple project. I need help integrating our google sign up form to google contacts. The fields we want to import is the business name, email, first name, last name, phone # and regions of work which are 4 options in the form. If possible, i would also like the regions to imported in their regions label. I found a script online to create this but i wasn't able to make it work.
