Full time work for developer with ZERO Other Projects currently

We are looking for someone that has vast knowledge of Laravel, PHP, AJAX, APIS, and many other technologies, to work for us full time on many different projects.
This first project is a simple test in which you will demonstrate your communication and coding ability. It is not paid for but if you complete it accurately, you have a chance to work with us full time.
In the attached page we have a form that collects data from the user and fetches it from google maps api. As the user types in the business name or address it populates and then inserts that data into the rest of form so that it doesnt need to be filled in. All the fields work fine except for the phone number field isnt being filled in. Your job is to make that populate in just like the rest of the form while making sure that the auto phone number field format stays in tact.
The api data has been removed from the file for security reasons, so you wont be able to test with our api, but feel free to test with yours.
This project is not paid but used as a method to test your knowledge and skill. Please ping me if youre interested in doing this quick test to demonstrate your skill.
