Require someone to digitize a form and add several features.
1. For attached. Users need to fil this form online. Data is placed into a data base.
2. each user gets a unique QR code that will be printed on a card. The data for the card is generated from the database here. The QR code is printed on the database, this QR code can be scanner to link to the user data on the database to confirm.
3. the form needs also feature to allow user to add their photo.
4. the form also needs the user to be able to add additional documents to their entry. Max 5 entry.
5. The admin side needs to be able to export certain data to excel format and others to analyze.
6. individual entry from backend by admin needs to be able to extracted to pdf format. This to facilitate printing this to a card. If possible to export data directly into the card layout would be must better also. So to faciliate the ID card printing process.
