Form builder in Angular with Laravel

READ THE ENTIRE DESCRIPTION BEFORE BIDDING.

I need to create a dynamic form builder with Angular and Laravel. The system must allow drag & drop fields or sections and generate a JSON to form the form with Formly (https://formly.dev). Each form response will be stored in a repository within the system and must allow the information to be exported to Excel.

All the basic fields must be considered:
- Inputs
- Textarea
- Date
- Check box
- Radio
- Select

Each field has validations and conditions can be added in each one (see Formly documentation).

The project should be well structured and ready to add more Formly features in the future (wraps, stepper, tables).

The goal is to create a tool similar to Google Forms and SurveyMonkey.

You must consider the CRUD of:
- Users
- Roles and permissions
- Repositories
- Forms
- Customers

Observations:
- I have no design
- I have my own server
- It will work with Angular and Laravel
