Fix installation of Opencart extension

I bought an Opencart extension to Opencart 2.2.0.0 that adds an insurance checkbox on the checkout page where users can tick and add insurance to their order.
I managed to install it and added 2 types of insurance in admin, but only one is shown on the checkout page.
Need someone to check it and add this.
