Fix Application Form on PHP Website & Test Payment methods

I am looking for an experienced and trustworthy website developer to do a small task.
We have an application form on a PHP website. The application form does not work properly and shows an error. The payment methods must be tested so we can see if the full application form works 100%. Testing the payment integration is important. It was previously done already...I just want it to be tested and remove any errors or processing issues.
Also, we want the application form to be sent to a different email address.
We want someone who is efficient, knowledgeable, patient, and professional.
Testing is important and all old code that does not work or not useful must be removed.
