Fillable Form to populate multiple Letter Templates
Budget: $30 – $250 USD
I'm looking for a professional who can create a fillable form with 15 fields that can simply and efficiently populate 1 or 2 of 4 different options of letter templates.
I want to be able to fill out 1 form that can create 2 letters from a choice of 4 different letter templates.
There are a total number of 48 blanks in all 4 letter templates which would be filled from the 15 fields on the form. (only 2 templates would be used at at time)
The form should include options that would determine which of the 4 letter templates is populated.
This should be simple and user friendly.
