Export form to an Excel (Reading Mysql info)

I am looking for someone to help me export form data from my website to an Excel sheet. I only want certain fields included in the sheet, 5 to be spesific. I require specific formatting and organization in the Excel sheet. Ideal skills for this project include knowledge of MySQL and Excel. Experience with exporting data and formatting in Excel is also preferred. Thank you for your interest in my project.
