Excel Task - Name Manager - Batch entry of Name and Refers To -- 2

Small excel task / vb script to create batch 'Names' and 'Refers To' in the Name Manager section from a table I have provided.
Excel Sheet (name manager xls) is attached and provides in table showing each of these
and have also added the Excel sheet (Final Call Forward) that these entries need to go into.
I have also provided an image with example of where it is and how they are entered.
As this is a small task I will offer $35 AUD to complete this.
