Entering data automatically into a PDF form

I am looking for someone that can take raw data (whether in an excel sheet, word document for example) and to enter it automatically into a PDF form. This PDF form has both STATIC fields and drop-down fields. It is a governmental form and has a "Validate" button that creates a bar code once the form is filled.
As such, the exact provided PDF file must be filled and you cannot generate a new PDF that looks esthetically the same because it will not be able to create the bar code once the form is filled.

As you can see there is a button called "Validated" on the top of page 1. when you complete and finalize the form and press Validate, then a bar code is automatically generated (see example in uploaded File).
