Enhancing Lead Form Functionality in existing CRM (Required: Experienced Laravel Developer)

Background - We have an existing CRM built on Laravel (up & running) and we would like to enhance its leads functionality. Please refer to the attached images 1 & 2 of how it looks currently.
Requirements - As stated in the requirement images. Non-negotiables. Also, we would like to incorporate Lead Scoring capability. (As mentioned in 3,4,5, and 5.1). Also, an e-signature must be compliant with the US - e-sign Act and UETA (Uniform Electronic Transactions Act), European Union - eIDAS (Electronic Identification and Trust Services Regulation), and other countries' laws.
