EXPERT Developer Needed to create a medical notification report
Budget: $30 – $250 USD
Looking to create a simple form where I can input information that will create an email notification to the user with the PDF attached with the answers I put into the form.
This is for a medical business.
We currently use JotForm pdf editor to generate pdf's from forms.
But we are having many issues as the email notifications are not being sent out with the PDF attachment.
I'm trying to recreate the functions that jotform gives me.
A simple form that a medic can fill out with a patients results like this: https://form.jotform.com/211809039691157
And the form sends out the results to the patients, with a designed PDF attached with the filled out form information.
Then in the backend i need to be able to keep track of the reports created and edit in case there's any typos just like in the backend with jotform and resend to the patient or be able to download the pdf report.
We have different types of tests that will have different form pages and fields and info different types of pdfs.
Backend needs to be password protected.
Need to be able to create access for different types of users.
How will it take you to create this?
Speed is very important to me.
