Dynamic HTML form with PHP and MySQL
Budget: $10 – $30 USD
Looking for someone to create a dynamic form that inserts data into a MySQL database.
The form will need to have toggle switches and based on the true/false value it should ask additional questions, some parts of the form should have the ability to insert additional results and ability to remove results with a plus/negative symbols. The result of the form should insert data into an SQL table or multiple tables which may require to use SQL joins.
The form should use pure javascript or jquery or a combination of both.
The inserted data should also be easily modifiable as well.
Furthermore, the final result (of all records in the database) should output into a JSON format in a separate file such as json.php or api.php (doesn't really matter) but the JSON result must be nested in a specific format (will be provided if job is awarded)
