Dynamic form builder is a web base application that's allow us to build our own form on spot (drag & drop) with all HTML5, JavaScript, PHP. following features should be considered:
1- Attributes can be made available once edit button is enabled and side menu will appear for selecting different type of fields such as text box, radio button, check box, image, file upload, label, etc.
2- Fields can be re-order by drag & drop.
3- Any additional fields added it show than be save as final template for that form and fields should take place in MySQL also.
4- Updating existing form also it should be possible by add/delete any attribute in the form.
5- Adding header also it should be possible and any attribute under that header can be visible/hide by clicking (+) icon.
6- Form can consist of many pages by navigating with next button.
7- Re-ordering of pages should be matter of drag & drop pages in list of pages.
8- Each station (page) should have option of action to be executed by landing/leaving the station such as sending SMS, email, WhatsApp, notification.
9- User can use different template to view dynamic form by applying ready template with CSS files.
