Duplicate simple AJAX form website
Budget: $250 – $750 USD
I have a URL I would like to duplicate the functionality. It is 5 parts, and really just a simple AJAX form with a signature box on the final page:

The signature component would simply plug/play this open source product so the viewer can sign their name:

I'm looking to have this form submit and, if not too time consuming, input everything from the form into a PDF file with each field properly populating the PDF template, and the signature obviously inserted as an image in the correct area of that PDF. I've attached the template PDF to review, it is only the first 5 pages that are relevant.
The (wordpress) servers this will run upon uses PHP, and I'd like to keep everything written mainly in javascript and PHP or even as simple as HTML if that makes more sense. Generating the PDF would presumably need to be via PHP. Am seeking a 2-3 day turnaround at longest.
