Duplicate a JQuery form into PHP from an 3rd party URL

I have a URL I would like to duplicate the functionality. It is just a few parts, and really just a simple AJAX form with a signature box on the final page:
https://www.getcardpointe.com/lp/cardpointe-application/. I have the working signature source code already implemented, and also the PDF code working and implemented. Very little time is needed on that, the only time would be naming and mapping YOUR form variables to existing form variables
I currently have another working setup that takes a form submission and populates a PDF. I can probably reuse that myself as I understand PHP well but I'll provide those PHP pages and ideally you would do so. All that would be involved is basically mapping the form you create above with existing variable names, maybe 20-30 of them total.
I need the webpage to load fast and not be too reliant on giant CSS files and many different JS files. Some are fine and needed I assume, but my current website is overloaded with them and it's costing me visitors & sales.
