Develop simple backend for a form app

We currently have a web form that generates a PDF we would like to develop a backend for traceability. @ kinds of profiles: Admin and Agents (agent should be able to fill the form) and generate it.
Admin will have Informations agent name ....
This is and urgent project, you must be available to start working
