Develop a campaign landing page with some php and mysql programming

Hello, I want to develop a landing page for a sales campaign I am managing.
A simple system should be developed to work on an Linux CentOS, Apache webserver. Coded in PHP and with MySQL as the database.
In the next instructive you can see the steps to develop the landing page.
The summary is that we want to show the visitors some interesting information about the zip code where their business operate. Fore saying something like: “In your area there are 10 italian restaurants and 20% of inhabitants are 35 years and older with an average income of 60K a year”
In total there are 12 of this cases (extra information will be sent to interested freelancers)
All the data needed is available already online in a Database in several tables.
The idea is to show 3 random cases (if the visitor refreshes the page 3 others will appear randomly).
Below these cases there will be a question that says:
"How can we help you", which will have 4 options, each option will give you some package options:
Increase pickup and delivery online orders - include 5 packs
Get new clients - include 5 packs
Increase the average ticket per visit - include 3 packs
Increase visits and purchase frequency - include 4 packs
On the other hand, If the users aren’t interested in buying any of the packs. They have to complete a form to subscribe to the newsletter.
But, if the users choose any of the packs , they have to complete the form (name, phone and email) Optionally, they can check the box if you want to subscribe to the newsletter.
