Develop PHP Web Screen to Update/Add/Delete Records from a MySQL Table

The developer must me proficient in PHP, MYSQL,AJAX,CMS to develop a Web Screen that will do the following:
1) Search table for Records
2) Display them on the Screen
3) Allow Addition of New records
4) Allow a deletion of a current record
5) Allow editing of records
Please understand successful on time and on budget success will be given many more projects I require doing.
