Debug PHP Script on MySQL - cPanel

I'm experiencing database connection issues with my PHP script. I'm not sure if these problems stem from recent changes to my MySQL configuration, as I haven't kept track of all modifications.
As a freelancer, you will need to:
- Diagnose the root of the database connection issues.
- Debug the PHP script accordingly.
- Provide recommendations on any necessary changes to my MySQL database configuration.
I have full admin access to both cPanel and the MySQL database, so you will not be limited in your ability to make necessary changes. Ideal skills for this project include expertise in PHP, MySQL and cPanel, as well as debugging experience.
The problem is about creating an attachment. It seems that the files being uploaded are not being saved into the database. Only 5 supporting attachments are required, only one works.
