Customized Gravity Forms Plugin

I'm in urgent need of an expert to customize my Gravity Forms plugin. This requires the addition of custom fields where each radiobox or checkbox input option carries its own HTML description - an understanding of Wordpress, Javascript, PHP, and HTML is absolutely necessary.
