Customization Shopify Verification Checkboxes

I am trying to add this verification for 3 checkboxes I created in my register page. I want people to agree to 3 different terms and conditions when creating an account and for the information to be saved under the client page for when they accepted the check boxes (time, date and if possible IP). Is this possible?

Something like the pictures, I have already created the check boxes but I am missing the time date and IP part within the client page after they created their accounts on my website.
