I’m seeking an experienced professional to develop a fully functional, interactive form in PDF, Word, or Excel—whichever format best supports the required features. The form will include data input fields, dropdown menus, and potentially conditional logic. Although the visual design will be simple, the emphasis is on reliable and user-friendly functionality, as the document will be downloaded and used offline by others.
This is not an online or web-based form; the solution must be fully self-contained in a desktop-compatible file format (PDF, DOCX, or XLSX).
Deliverables & Functional Requirements:
• Design and implement a clean, user-focused layout based on the provided high-level mockups (attached).
• Embed interactive components (e.g., fillable fields, dropdowns, checkboxes).
• Ensure the form supports data persistence or automation, such as:
• Carrying over information from a previous form (e.g., last week’s “Action Items” auto-populate under “Previous Week’s Action Items”).
• Displaying fields like “Time of Last Connect” upon clicking a “Meeting Complete” button or similar event trigger.
• Include light automation or scripting if necessary (e.g., Word macros, PDF JavaScript, or Excel VBA) to enhance usability and reduce manual input.
