Custom Database and PDF Report Builder

I'm looking for a skilled developer to create a straightforward system that allows me to input data into a custom database and generate detailed reports at the end of each day.
Key Elements to Include:
• Space for twelve specific data fields: Name, Locality (dropdown), Call (checkbox), Time (timefield), No Determination (checkbox), Number of People (numeric field), Type of People (dropdown), OWI (Yes/No dropdown), and Remarks (free text)
• The 'Locality' dropdown should contain various location options, to be provided.
• The 'Type of People' dropdown should include three options: Adult, Child, and Mixed.
• At the end of each day, the system should auto-generate a PDF report from the daily entries.
Ideal Project Completion Time:
• As soon as possible.
Ideal Skills for the Project:
• Strong experience in custom database creation
• Experience with PDF report generation from databases
• Familiarity with a range of data input options (text, dropdown, checkbox, etc.)
