Custom CRM PART 1 Input Forms

Hi, I need a CRM built. I would like to build this from scratch so we can have full flexibility in adding exactly what we want over time.

I am going to split the project into 2 parts with further work beyond that. This project is for Part1 and will need to be completed this week by Friday.

Part 1
Input form
1. We will need a secure login (login page to be branded, nice design)
2. Customer Application, this is an input form, a series of sliding pages with questions, graphics, dropdowns etc that we will select the answers to and input information
3. This data will then be stored in the Database as a client record

Once Part 1 is complete we can then move onto Part 2, which will be processing the application data into outcomes.

I am after a skilled person in developing database / crms, Ideally I want to work with a small team or an individual so we can build and keep creating as we go along. Future work will build in complexity so if you are not skilled that please don't bid.
