Create 2 web forms that feed into Google Sheets and print out in a nice format & 2 Google Docs.

I am looking for a freelancer to create 2 web forms that will feed into Google Sheets and print out in a nice format, as well as 2-5 completely separate Google Docs.

The forms will be individual fitness and nutrition tracking sheets that will collect daily input of nutrition and exercise with comments from clients to track client activity so the trainer can monitor compliance and progress. This information will need to be fed into google sheets so the trainer can monitor the individual's compliance and progress, as well as the progress and compliance of the group of clients. It will need to flag or highlight clients based upon logged compliance, so the trainer can see where help is needed.

I'd like for the clients to be able to update these forms on their phones too.

Ideal skills and experience for this job include:
- Proficiency in web form creation and integration with Google Sheets
- Ability to design a clean and user-friendly layout for the forms
- Experience with data collection and tracking
- Attention to detail to ensure the forms and sheets are accurately connected and functioning properly.
