Create a multi-page registration form

I posted this project but the freelancer couldn't deliver the project so I repost it here.
I need a multi-page registration form like the following link in PHP and Mysql:

Please consider that you should create a sample record on that website so that you find out its workflow and required fields. I need all of the fields on that registration form. It's straightforward and there is no need to explain more requirements. Please consider that this form has files and photo uploads too. Also, there is a dashboard for managing the registration process.
I only read the bids that begin with the Rama word at the beginning.
Please pay attention that I only need the Spa registration sections. (not the Hotel, restaurant, Travel, and GM). Also, I don't need any section that is opened after upgrading (for the premium plan)
