Create PHP PDF Form Builder using existing PDF

If you have already this with code that will be good.

I am looking to create PDF Forms that have inputs like text, dropdown, checkbox, radio, date, signature etc.

I will upload the PDF file and it will show me the view and then ability to add above inputs in the PDF and then save it as pdf form.

purpose is that i have many types of pdf files that need to have input i want to send it to people who will fill the pdf form and send back to me.

Please let me know if you can do this job.
