Create Joomla form to osTicket using API

We recently started using osTicket to track our incoming support requests. I need someone to create a Joomla forms extension that will allow a user on my site to fill out a form and the information needs to be sent to osTicket for automatic ticket creation.
The email the user provides needs to go to acymailing as well for future emails.
If there is already a Joomla form extension out there that will do this, I will also pay someone to set that up for me.
