Create me a Form Like this Using WPForms Pro (I have a license)

Create me a form that looks and does the same as this with some small changes

- Use WPForms Pro - I have the license
- On submit saves record
- I use WPForms Pro
- Saves records to DB
- Send user to another page for checkout
- User needs to select a schedule date
- When user selects schedule it pulls up date and time for them to select
