Create Email Signup Page & Integrate Brevo

I need a sign up page created on a live website and connect Brevo to it( I already have a Brevo account)
The signup page will be email-only, requiring users to provide their Name and Email. It will also need to have additional functionalities such as Captcha verification, Email confirmation, a Welcome email, and an Automated email.

The following is the page that needs a sign up form connected to:
https://outchea.co.uk/sign-up-lagos
(I already have a sign up page but this is not going to be connected to the the sign up button that is on the site already. I need a new sign up page using the same style font, fields and buttons).
The following are the required fields:
- first name
- Last name
- Email
- Country code prefix in numerical or alphabetical order (I can provide you the script for this)
- The remaining number as a suffix
This NEW sign up page (https://outchea.co.uk/sign-up-lagos) will be directly connected to Brevo. Brevo will collect the data of each sign up.
An automated response email will also need to be connected. Each time a user signs up, they will receive an email. I have already designed the automated email in Brevo. All you need to do is connect it so it works.
The budget is $20
I need this completed within 3 hours
