Create a Donorbox style widget

I'd like you to create a similar Donorbox-style widget along with a php page which would allow me to select/customize the widget, which I could embed in our own domain. Parameters entered in the widget would be stored in a mysql database.
We are looking for a long-term relationship with the right coder and have several projects in the pipeline.
