Create Customer List in CSV from MSSQL Data table -- 2

To export customer details into a CSV file from a MSSQL table. Need single page PHP webpage that can search for customer (lastname, firstname) select from the results and add to customer list. Can add/delete/ re-arrange the order of customers in list then export that to a CSV file.
Sample connection given in PHP, same file from MSSQL table and Sample CSV file output. Need to be done in 1 day.
