I am looking for a skilled PHP/Web developer to convert my current spreadsheet to forms backed by a MySQL database. For the form building platform, I would prefer to use JotForm. As for the database management system, I would prefer MySQL. For the level of user access and security, I require intermediate access with multiple user access with restrictions.
